Any fighting, abuse, erratic behavior will result in my party from being ejected from facility. No drugs, paraphanalia, or smoking allowed in facility. Alcohol is NOT permitted. ANY violaion will result in removal from facility or law enforcement

I agree all guests must wear socks and are required to remove shoes.

I understand I am not allowed to bring glitter, confetti, sticky or dyed foods/drinks. No food or drinks allowed in play area.

I understand all of my guests are requird to sign a waiver, and any guests who fail to do so, I accept responsibilty for liability, behavior, injury and damages.

I understand I am required to leave the facility free of trash, debris, and personal items. I am required to spot sweep and spot mop floors, wipe tables and leave fridge in clean & working condition.

*FINANCIAL AGREEMENT* I understand I must pay a $50 damage deposit on or before party. I undestand I must pay 50% of charges to book party. I understand my party balance is due at least 24 hours before date of party. I understand only 50% is refundable if I cancel my event. I understand I will be charged for damages to chairs, tables, or other equipment. I understand I will be charged a cleaning fee if I do not clean party room. I understand I will be invoiced for charges that exceed my deposit.

I understand I am not permited to show up early or stay late without proper reservation and payment. My time includes set up and clean up time and I am solely responsible for managing and using that time properly so it does not interfere with othe parties or business functions.

I hereby acknowledge this release from liability for accidental injury or illness which I may incur as a result of participating in any physical activity. I hereby assume all risks connected therewith and consent to participate in this program. I agree to disclose my physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in this program.
